Services
Individual & Couples Telehealth Counseling
I offer individual and couples counseling for Ohio residents via a secure telehealth platform. My specialities include anxiety, depression, trauma, substance misuse or addiction, LGBTQ+ issues, and grief. I am currently in-network with several insurances and accept self-pay clients.
Supervision & Consultation
I offer consultation and supervision services for provisionally licensed professional counselors. I use a feminist, strengths-based approach in supervision with a focus on shared therapist and client liberation and the common factors model of counseling.
$50 per supervision hour
My services are available to every person regardless of race, ethnicity, gender identity, gender expression, sexual orientation, class, ability, national origin, military status, religious or spiritual practice, marital status, age, or citizenship status.
Fees & Policies
I am in-network with the insurance companies listed below. If you are using your insurance for services, please contact your insurance provider prior to your appointment to determine your deductible, co-pay, and limitations for covered services. If your insurance does not cover services, you are responsible for payment. If I am not in-network with your health insurance provider, you may still be eligible for partial or full reimbursement for out-of-network services. Self-pay Rates:
60-minute initial intake: $150
60-minute individual session: $120
60-minute couples session: $140
Co-pays or payments are expected at the close of each session. If you are seeking reimbursement for self-pay rates through your insurance, I may be able to provide you with a superbill (a document outlining the services provided that you can submit to your insurance) at the end of each session.
In order to provide timely services for all clients, I do have a late cancellation and missed appointment fee. If you need to cancel or reschedule your appointment, you must let me know at least 24 hours in advance. If you cancel or reschedule less than 24 hours before your appointment or miss an appointment, a $65 fee will be charged that is not covered by insurance. In addition, if you are more than 15 minutes late to your scheduled appointment, that will be considered a missed appointment.
Good Faith Estimate
Federal legislation passed in 2020 protects patients from unexpected costs for healthcare services. The No Surprises Act (NSA) gives patients the right to a Good Faith Estimate (GFE) of the cost of services provided at Canyon Counseling & Consultation. Under this law, healthcare providers are required to provide a Good Faith Estimate of the expected cost of treatment services to individuals who are not enrolled in a healthcare insurance plan or prefer not to use their healthcare insurance for any reason. Regardless if you use insurance or not, the Ohio licensing board requires me to provide you with the costs of my services in written informed consent prior to providing services. Until it is determined if you will use insurance or self-pay, the information on Good Faith Estimates is based on listed self-pay rates. If you do intend to use insurance, check with your insurance carrier to determine your deductible, copay, or coinsurance rates, as they may differ from my rates.
The Good Faith Estimate will show the costs of services that are reasonably expected for your healthcare needs. Since your symptoms and needs have not been assessed, the estimated course of treatment, and therefore cost, is difficult to determine. Thus, the Good Faith Estimate is based on the information known at the time it was created and does not include unknown or unexpected costs that may arise during treatment. Any changes to my fees will require a new Good Faith Estimate and change in the informed consent, which you must agree to prior to them going into effect. You are free to discontinue services at any time or the services may be terminated in accordance with informed consent.
The NSA states that you may initiate a dispute if the actual charges are substantially in excess of the Good Faith Estimate provided (if you are charged $400 more than the estimated cost for a session or for the total estimate provided).
